Dance Camps #1 June 25 - June 29 - 10am-12pm $100 ($90 Early Registration)
#2 July 9 - July 13 - 10am-12pm $100 ($90 Early Registration)

Two Week Ballet Session (7 to 9 years)
June 11 - June 22 - New students or students currently enrolled in Ballet 2, 3 or 4.
Mondays & Wednesdays 9-11am $100 ($90)

2 Week Ballet Session for Pre-Pointe and Pointe
June 11 to June 22 For new students or students currently enrolled in Ballet 5 or Ballet 6.
Mon/Wed/Fri 10am-12pm $125 (Sign up and pay in full by May 1st you receive a 10% discount)

Drama Camps June 11 to June 15 - 1 Week Camp. 5th to 7th graders (Fall 2012). Mon to Fri from 12:30pm to 2:30pm Tuition: $100
June 18 to June 29 - 2 Week Camp. for Junior High students 8th, 9th and 10th graders (Fall 2012) Monday thru Friday from 12:30pm to 3:30pm - Tuition: $250

Summer Intensive Level 1
July 9th to July 28th - 3 Week Program. Tuition is $425. If you sign up and pay in full by May 1st you receive a 10% discount. If you are a new student to EPAC there is a $50 registration fee. $75 non-refundable deposit is due with registration form and is applied to tuition. Full tuition is due by July 2nd.

MEDICAL CONSENT: In the event of injury, I hereby authorize the program officials of the Eagle Performing Arts Center to arrange for medical services as may be deemed reasonable and necessary to the welfare of the injured, and I do hereby release the Eagle Performing Arts Center and all others from all liability in taking such action, including all action which maybe contrary to personal religious beliefs. I, the undersigned, have read this Release and Consent to medical treatment and understand all its terms. I execute it voluntarily and with full knowledge of its significance. LIABILTY RELEASE: I do hereby agree to release the Eagle Performing Arts Center and all other cooperating agencies, employees, officials, or managers thereof, from all liability for damages by reason of injures or property damages that may be sustained as a result of participation in this program. PHOTO RELEASE: I, the undersigned, hereby give Eagle Performing Arts Center, its agents, and/or assignees permission to use the photographs, motion pictures or any reproductions of my physical likeness taken of me in any manner it deems proper. I relinquish all rights, title, and interest I may have in the finished pictures, negative, and copies. I waive the right of prior approval and hereby release Eagle Performing Arts Center, its agents, and/or assignees from any and all claims from damage of any and all kinds based on the use of said material. I hereby warrant that I am a parent or legal guardian of the subject of photography, who is under eighteen years of age, and am competent to act in his/her behalf insofar as the above is concerned.
